Im guessing that the icon over this NPC's head mean's they have enhanced perception. Ive run into it before on hostile mobs and they seem to see me while stealthed even from far away. Also, I only see the icon while Im stealthed.

Anyone want to confirm?

NPC Stealth Detection
Certain Enemies are immune to stealth. These are normally extrasensory beasts (hounds/wolves who can smell/hear the player), Extremely powerful boss characters, and certain magical creatures. These anti-stealth NPC's can be recognized by a symbol over their head, of a sphere with a roving eye. The symbol appears once you enter stealth, but is not visible when out of stealth. These creeps will thus react to you as if you were not stealthed, and aggro normally.
